Wake Forest University School of Medicine received a $200,000 grant from The Leon Levine Foundation for continued support of CarePlus, allowing for the integration of a nationally recognized Collaborative Care Model as part of the program.
The CarePlus model of care at Atrium Health Wake Forest Baptist Downtown Health Plaza provides care to a population of patients with complex medical, behavioral and social needs. The grant will bring a licensed clinical social worker and a community health worker on board to assist patients with care navigation, helping address both physical and mental health needs during a single visit. The Collaborative Care Model is nationally recognized as a best practice for meeting patients’ behavioral health needs in the primary care setting.
“The impact of this support ensures we can care for the patients’ social and behavioral needs as we partner with them on their wellness journey,” said Dr. Richard Lord, program director for CarePlus and chair of family and community medicine. “The Collaborative Care model will help us extend the goal of hope, health and healing for all to some of the most vulnerable patients that we serve as Atrium Health Wake Forest Baptist.”
