Wake Forest University School of Medicine has received two grants totaling $300,000 from The Leon Levine Foundation. These grants aim to strengthen the CarePlus model program, which provides primary care and care coordination for approximately 500 frequently admitted patients from safety net practices, many of whom are covered by Medicaid or are uninsured.
The first grant will expand programs from Atrium Health Wake Forest Baptist’s Downtown Health Plaza to the health system’s Winston East facility - recently renamed the Kennedy-Hopkins Medical Building - to further improve patient outcomes in underserved communities. As the health care landscape continues to evolve and transition to embrace new models that screen and identify social determinants of health, the CarePlus model offers primary care and care coordination for approximately 500 patients at the Winston East clinic who are covered by Medicaid, along with uninsured patients with a history of recurrent hospital admissions.
The second grant will support the Downtown Health Plaza’s CarePlus model by improving maternal wellbeing, encouraging more prenatal care participation, and encouraging WIC enrollment for better maternal nutrition. Counseling by a licensed clinical social worker will help address the social determinants of health for women and promote healthy pregnancies. The CarePlus model represents a transformative step toward reducing the prevalence of pre-term deliveries.
