Health Care Providers Join Efforts to Meet Required Health Assessments and Vaccinations for Winston-Salem/Forsyth County Students

September 26, 2016

Wake Forest Baptist Medical Center, Novant Health and School Health Alliance for Forsyth County will offer special accommodations this week to help Winston-Salem/Forsyth County students meet state health regulations required to stay in school.

In addition to vaccinations, a new law requires students entering North Carolina public schools for the first time to have a health assessment form filled out within 30 days of school entry. This applies to students who move from other states or countries as well as students who move to the public school system from a private school or home school situation. The health assessment is not required for children who move within the North Carolina public school system.

To remain in school on Thursday, September 29, kindergarteners will need a health assessment and proof of vaccinations, 7th graders will need proof of vaccinations, and all students who are new to Winston-Salem/Forsyth County Schools will need both a health assessment and proof of vaccinations.

Families of Winston-Salem/Forsyth County students who have not met this requirement and have a pediatrician or primary care doctor are advised to call to schedule an urgent appointment letting the scheduler know that your child needs to be seen for a health assessment and/or vaccination to stay in school. Families without a primary care provider should know that the following health care providers are offering special accommodations, expanded hours and open appointments at the below locations:
